* For EAP benefits, you will need to provide the following information:
-We are happy to provide a "superbill" receipt after each appointment, which can be submitted to your insurance company for OON claim reimbursement, assuming your plan has OON benefits.
-In addition, we've partnered with Mentaya, a service that streamlines getting reimbursed for your therapy sessions through out-of-network benefits.
Here's how it works:
*Mentaya charges a 5% fee per claim, which includes handling any paperwork required, dealing with denials, and calling insurance companies.
See if you qualify: https://www.mentaya.com/patients
-Payment is respectfully due at the beginning of each therapy session.
-All fees for services rendered are the sole responsibility of the client.
-Payments can be made using all major credit cards or debit cards.
-HSA and FSA card payments also accepted.
-Cancellation with at least 24-hour notice: No charge.
-Cancellation with less than 24-hour notice or missed appointment (no-show): $75.
Arriving late will shorten your session, as it will still end at the scheduled time. We allow a 15-minute grace period for late arrivals, during which time you're welcome to join. After 15 minutes, the session will be considered missed and will need to be rescheduled, out of respect for other clients and our time.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item.
You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call Graceful Mind Therapy at 281-394-1161.
